The application must be filled out completely and returned to the campus principal as soon as possible for immediate processing. Note: incomplete applications will create a delay in the child’s participation in the program. Klein ISD students who were in the program during May 2012 will continue to be enrolled when school starts, however, will be deleted from the program if a new application for the 2012–2013 school year is not submitted within 30 business days from the first day of school.
The application must include the following:
- total household income or SNAP/TANF case number
- names of all current Klein ISD students with grade level and campus assignments
- names of all household members
- signature and the last four digits of the social security number (or the word “none” if not available) of one adult household member
