1.Proof of Identity PK-1st graders must have original birth certificate; all other grades may present birth certificate or another form of identity, such as a passport or social security card.

3.Proof of Residence This may be a lease agreement or a light, water, or gas bill with a parent’s name and address displayed. Telephone bills are not acceptable. Also, it must be current. Bills that are out of date are not acceptable.

4.Student’s Social Security card This is used by the state for identification purposes. It is not required; however, should issues arise with your child’s Social Security number, you will need to provide the card for verification. Therefore, it is better if the campus already has a copy on file. If your child does not have a Social Security number, then a State ID number will be assigned.

The following registration forms must be completed and signed for enrollment. Please follow the links, fill out the form completely and print it out. Be sure to sign each form and bring them with you to the school when you register your child.

Pre-kindergarten – A child must be 4 years old on or before September 1st of the current school year

Kindergarten – A child must be 5 years old on or before September 1st of the current school year

A child who is 4 years old on or before September 1st of the current school year may attend our half-day Pre-kindergarten program if they meet at least one of the following criteria:

• Unable to speak or comprehend the English language as determined by a test administration
• Is Educationally Disadvantaged based on income documentation
• Has a parent who is Active in the Military (must show documentation of such)
• Is Homeless (must fill out documentation forms)

• Do I have to use On-Line Registration?
o No. You may elect to complete the registration paperwork provided by your campus.

• Is my child’s information secure?
o Yes. These registration forms do not transfer data over the Internet. If you receive the message “Uploading Data”, it means that it is uploading the completed form to your printer.

• Does this mean I won’t have to go to the school to register my child?
o No. This is just to give you a head-start on the enrollment process. You will still need to take all the registration forms and documents to the campus.

• Do I need a printer for this?
o Yes. This site does not transfer data over the Internet to the campus. These forms must be printed out and brought to the campus for registration.

• Can I email the forms to the school?
o No. The schools can only accept paper copies of these forms. Please print them out and bring them to the campus to register
